Medication On-site PV Day Camp

Hello All, NO CHILD is permitted to carry ANY type of medication at Putnam Valley Day Camp. If your Child needs to have any over-the-counter (must be new and unused and not expired) or prescription medication this form (attached) MUST be filled out. All medication is to be stored with the Nurse at all times, not in your child’s bags.
