PV Day Camp Message
Hi All,
We need your childs physicals and immunization records over here at Parks and Recreation if they are attending our day camp. Get them in please!!!
Your child cannot attend camp per the Department of Health without an up to date physical/immunization record. They check the records every summer!
You can fax them 845-306-2114, mail them 265 Oscawana Lake Road Putnam Valley NY 10579, or drop them by the Rec office we are here 8 am-4 pm or there is a drop box on the wall under the covered porch to the left of the front door.
