Final Payment and Physical and Immunization Records Due June 3rd 2024
Hello PVDC Families,
This is just a reminder that we need:
UPDATED physicals and immunizations for your campers & Final payments made BY June 3rd.
Ways to send Physicals and Immunization records in:
1. Email them to Tcortina@PROTECTED or Aspinelli@PROTECTED.
2. Fax them to 845-306-2114
3. Drop them off at Parks and Recreation our office is open 8am-4pm
4. If you chaild needs mediaction at camp I have attached the form the doctor needs to fill out in order for us to Administer medicaion at camp.
How payments can be made: (all Creidt cards are charged a 4% fee)
1. Payments can be made on line at PVPR Registration Site (PayPal) Accepted credit cards-Mastercard, VISA,Discover and American Express
2. At parks and recreation in person or over the phone (845)526-3292 our offices are open 8am-4pm (cards accepted over at P&R-Mastercard, VISA and Discover.
3. We have an afterhours box at parks and recreation on our covered porch.
