Good Morning,

I am looking for all children who have been vaccinated for COVID-19 some of you have it listed on your child's physicals. This is incase there is an outbreak, if there is one, I need to povide the health department with how many children and staff are not vaccintaed vs vaccinated. I have to do the same with the Polio vaccination, but everyone has this on their physicals.
