Physicals and Immunization Records
Hello Day Camp Families,
We are looking for updated Physicals and Immunization records. You can email them to Tcortina@PROTECTED or you can Fax them to 845-306-2114.
If your child needs medication on site this summer I have attached the form for the doctor to fill out. We will need the medication in the original container with the prescription label on it; if it’s over the counter medication we just need it in the original container.
All Physical, Immunizations and Final Payments need to be in no later than June 2nd!
