PV Day Camp Message
Hi All,
We are still looking for immunization records and physicals for some campers. Our Camp Nurse is reaching out by telephone if we are missing your child's records. CHILDREN MAY NOT ATTEND OUR DAY CAMP WITHOUT AN UP TO DATE PHYSICAL AND IMMUNIZATION RECORD ON FILE!
Your physician can fax the forms directly to our office-845-306-2114.
There are some outstanding balances still to date. If you have an outstanding balance, you will receive an invoice via email within th...e next day or two. All balances must be clear by the start of camp unless a special arrangement has been made at the Rec Department.
On the first day of camp, please escort your child to the PVPR table and we will make sure your camper is all clear to attend!
